Preoperative Consultation
Associated Oral & Maxillofacial Surgeons
of Peoria believes consultation prior to surgery allows the patient
an opportunity to meet their surgeon and review any questions or
concerns. In addition, consultation provides our doctors the
opportunity to review your medical history and address any
considerations or limitations certain medical conditions may
impose.
All patients undergoing anesthesia, hospital based procedures,
who are taking certain blood thinners, or may have serious medical
conditions are required to schedule a consultation prior to
surgery.
Patients having minor procedures that are performed in the
office under local anesthesia may defer the consultation
appointment.
Patient Registration
You may preregister with our office by filling out our secure
online Patient Registration Form. After you have completed the
form, please make sure to press the Submit button at the bottom to
automatically send us your information. On your first visit to our
office, we will have your completed form available for your
signature. The security and privacy of your personal data is one of
our primary concerns and we have taken every precaution to protect
it.
